Tennessee Department of Correction Commissioner Frank Strada resigned after death row inmate Christa Pike survived two lethal injection doses, Governor Bill Lee said. Lee paused all state executions and appointed former U.S. Attorney Ed Stanton to conduct an independent review of the department's actions in carrying out Pike's court-ordered sentence.
